I like the art work on the thumb. It just kind of jumps out at you. But the one thing I wouldn't like are the placement of the pills in the corners. Taking those pills would be annoying at best.

There is a little pb of symmetrie in the extreme right, and only 4 bases are in pills range... The rest looks good ;)
